I invite you to psychotherapy and coaching that is free of external constraints (like, e.g., those posed by insurance companies) and that is evidence-based. This allows us to focus on what you want to work on rather than on third-party demands. This also allows us to transcend the narrow focus of traditional psychotherapy as fixing pathology and expand the applications of effective intervention to personal growth and goal achievement that you define. Only you and I decide how the process is conducted, and you can always change your mind, choose more traditional approaches, and you will receive appropriate referrals.
What that means is that I want to have a thorough understanding of your situations and your emotions before we set the therapy agenda or start using any methods of change. You will also learn all the elements of empathy to apply in your relationships if you choose to do so. The important expression of this empathy is testing (T in TEAM CBT) that is used before and after each session. This way, in a quick way, you can succinctly express how you feel at that moment, and we can track your progress in the course of the session as well as between sessions. This keeps us accountable. Also, after each session, you will provide your honest (I hope) impression of me as your psychotherapist, the process of the therapy and its effectiveness, therapeutic alliance, as well as what you liked the least and the most about the session.

Which means we will not focus on the different DSM IV or ICD 10 diagnoses but on you and your problems. Your diagnosis has value, and I will obtain detailed psychiatric history, but my approach is to help you with the issues that are most pressing for you. When you learn effective ways of resolving your current most pressing problems, you learn skills for addressing other problems. These skills will generalize to effectively resolving difficulties in other areas of your life with or without my help. The process of my therapy is transparent, and you will learn all of its aspects. You will be able to choose the methods we will apply in your therapy. In fact, you will learn how to become your own counselor and psychotherapist and apply these skills to yourself as often as you desire to intensify the therapy and increase its effectiveness.
I am not against psychiatric medications, but I noticed that if I use it together with psychotherapy, they may disturb the focus of our sessions and impede the therapeutic process. I don’t mind if you are on mood stabilizers, antidepressants or antipsychotics that are prescribed by another psychiatrist and are not the subject of our sessions. Also, if you are chronically using addicting medications like benzodiazepines (Xanax, Ativan, Valium, etc.) or sleep medications like Ambien, Sonata, or Lunesta, I could help you get off them if the treating psychiatrist agrees with that. The evidence says that these medications interfere with cognitive-behavior therapy effectiveness.

I like the intense therapy in 2.5-hour sessions. Such extended time allows for providing enough empathy to achieve deep enough insights and the application of effective methods in the same session. You may choose to first use 1-hour sessions before going for the extended 2.5-hour one. See pricing below.

For the rapid resolution of symptoms, homework is not negotiable. You will be required to do therapy homework (reading, writing, engaging in behavioral assignments, etc.). If you don’t plan to do them, don’t schedule even the 15 min free orientation appointment.

With those who are interested, I also apply Cognitive-Behavior Laughter Therapy and Cognitive-Behavioral Non-Denominatinal Spiritual/Existential Counseling with elements of nonduality.

Mariusz Wirga, M.D.

I am a psychiatrist, cognitive behavioral therapist & CBT trainer, and psycho-oncologist. I am a co-author (with Maxie C. Maultsby, Jr. and Michael DeBernardi) of ABC of Your Emotions, a 340 page book with 280 cartoon-like illustrations explaining our emotions and how to manage them effectively. I serve as the medical director for the Psychosocial Oncology program at the MemorialCare Todd Cancer Institute and the Chair of Psychiatry, at Long Beach Memorial Medical Center. For over 35 years, I have been refining ways of addressing the emotional, psychological, interpersonal, as well as the spiritual or philosophical needs of participants in my classes, workshops, groups, and individual sessions. My primary focus has been working with patients affected by cancer and their families.With Dr. O. Carl Simonton teaching 1998.

Pioneer of Psychosocial Oncology

O. Carl Simonton, M.D.

Dr. O. Carl Simonton was a world-famous physician, radiation oncologist who earned his fame through his pioneering work in exploring and addressing the psychological, interpersonal, and behavioral factors in recovering from cancer. In April 2021, in multiple places around the globe, there were celebrations of the 50th anniversary of the Simonton Program. People from Japan, Poland, Germany, Italy, Austria, South Africa, and the USA reminisced on its founder Dr. O. Carl Simonton, and how his work and philosophy affected them.

Born on June 29, 1942, in Los Angeles, California, he dedicated his life to the development and refinement of interventions for cancer patients and their families. 

Therapy

Single-session therapy

We are focusing on what we are going to accomplish in one 2.5-hour session. Sometimes, that one session may be everything the person needs, and even in chronic problems, a profound and lasting transformation may occur. We don’t count on achieving such excellent psychotherapeutic effects, yet with each encounter, we are open to the possibility that it may be the session that will bring the resolution of symptoms. It may be followed by a consolidation and relapse prevention session, supplemented by booster sessions if needed. There is growing scientific evidence for the effectiveness of single-session therapy and the possibility of sudden gains in CBT, even with treatment-resistant depression. Such rapid and long-lasting results are possible with the pre- and between-session use of homework, bibliotherapy – reading books and book chapters, listening to podcasts, etc.

I also provide intensive therapy sessions - multiple 1 to 2.5-hour sessions over 2-14 days (including twice a day).

The therapy progress is assessed on a continuous basis. If there is no desired progress in the weekly therapy, collaboratively, we explore and address potential factors. The evidence shows that the most progress is achieved in the first 6 months with a new therapist, so I like to limit the therapy to 6 months. Yet, I have patients in therapy that see me a few times a year or every few years for crisis interventions or boost sessions.

Unfortunately, your health insurance doesn't pay for such a form of therapy, they dictate the frequency and length of therapy, they require that you be given a psychiatric diagnosis, they may demand psychopharmacology management, and they can have access to your records.

I am also committed to giving these values first priority in therapy and acting as a tireless proponent of anti-racism and anti-oppression principles in my professional life, all of my personal connections, my neighborhood, and on a societal level. At the same time, I am aware of my privilege and that I have blind spots and work at eliminating them, and that is my never-ending assignment.

  • I provide therapy primarily via HIPAA-compliant Zoom-based service - in-person sessions primarily when applying in vivo exposure or another form of intervention requires it.

I found that my approach is not effective with someone who:


  • Doesn’t do psychotherapy homework and assignments between sessions

  • Actively uses addictive substances or is dependent on them

  • Has significant developmental disabilities or cognitive deficits that impair new learning,


Therapy with me may be helpful to people with schizophrenia or bipolar disorder if they are stable on their medications. Active substance use or dependence may be effectively treated with CBT and other modalities, so I would recommend to seek a therapist specializing in it (I am not a substance abuse therapist).
Also, since we focus on progress in psychotherapy, please don’t ask me to fill out any paperwork, including but not limited to paperwork for accommodation at work or school, disability, FLMA, emotional support animals, etc.
